Spring sports packets due no later than February 25
Interested in playing a spring sport at GHS? Now is the
time to sign-up! Spring sports include boys/girls track and field,
boys/girls lacrosse, boys tennis, softball and baseball.
Students interested in playing a spring sport should pick up a sport sign-up packet from the high school nurses' office and return the completed packet to the nurses' office as soon as possible, but no later than February 25, 2014. The sport packet includes three forms: the School and Sport Physical Form, the Athletic Form and the Health History Form—all of which are also available online.
Students needing sports physicals should schedule a time to stop by the high school nurses’ office on either:
• Monday, February 10, 2014 from 4 – 5:45 p.m.
• Monday, February 24, 2014 from 4 – 5:45 p.m.
Please note: Students are encouraged to sign-up for their physicals in advance at the nurse's office. Appointments may take up to one hour, so please plan accordingly. If applicable, please bring glasses or wear contacts for the eye exam. Students may have physicals done by their own doctor, but the physical must be completed on the GHS physical form and reviewed by the school nurse before a student can sign-up for a sport or attend practice. Physicals are also open to all students for working papers, tenth grade physicals, and new student physicals.
Students will not be able to try out or participate in practices without a sports packet cleared by the Nurse’s Office. The Department of Athletics form and the Health History form should be handed in ASAP; the Physical Examination form must be handed in before try-outs/practice starts.
Please note: If a student is seen by a doctor for any injury, accident or serious illness, the parent/guardian must notify the school nurse. This is required whether or not the injury/illness occurred on school grounds. Upon the student’s return to school, after seeing a doctor, the student must present to the school nurse either a written release or a PE Program Modification Form, filled out by the doctor. The school nurse will review all medical notes and copy the appropriate staff (PE and coaches). The original note is kept on file in the Nurse’s Office.
