Fall sports offered at Guilderland High
School for the 2008-09 school year include:
Students interested in playing a fall
sport should pick up a sport sign-up packet from the
nurses' office and return completed packet to the nurses'
office as soon as possible but no later than Friday, June
13, 2008.
The sport sign-up packet includes three forms: the School and Sport Physical Form, the
Athletic Form and the Health History Form.
Each student needs:

1. |
A physical within 12
months of the beginning of the sport season he/she wishes to play.
Student can go to his/her own physician for a physical or have
one done by the school physician. Students should go to the nurse's
office to check to see if they have a valid
physical. All physicals MUST be completed on the
official GHS physical examination form;

Students needing sports physicals,
school physicals (new students and 10th grade physicals) and/or physicals to obtain working papers
should make an appointment with the high school nurses'
office for either:

Wednesday, June 11, 2008 from 4 - 6
p.m.

Thursday, June 12, 2008 from 4 - 6 p.m.
Please note: Physicals are by
appointment only and may take up to one hour so please
plan your time accordingly. Also, please bring glasses or wear
contacts for eye exam, if necessary. Students may also
have physicals done by their own doctor, but the physical
must be reviewed by the school nurse before a student can
sign-up for a sport or attend practice. |

Students will not be able to try out or participate
in practices without a sports packet cleared by the Nurse’s
Office. The Department of Athletics form and the Health
History form should be handed in ASAP; the Physical
Examination form must be handed in before
try-outs/practice starts.
If a student has an injury which impacts his/her
participation in interscholastic athletics, or PE, the
parent/guardian should notify the school nurse. Upon his/her
return from an injury, the student must have either a
written release or restriction from the attending physician,
which should be given to the school nurse for review,
a copy will be sent to the Coach, and the original kept in
the student’s health records.
If you have had a valid physical within
the past 12 months and it is on file at the nurses' office,
only a completed health history and emergency athletic form
signed by a parent is required to be eligible for a spring sport, provided all injuries have been cleared by a
physician.
