State required physicals

State Education Law requires that all new students and students entering grades kindergarten, 2, 4, 7 and 10 must have a physical examination by either their family physician or the school physician. It is recommended that your family physician, who is most familiar with your child’s medical history, perform the physical, and return the physical examination form to the appropriate school's health office.

In compliance with the law, our school district will provide for the required examination by a school physician for all students who do not return evidence of a physical examination by the family physician. Please call the nurses’ office at the school your child attends to obtain information on the physical schedule and a physical form if you are seeing your own physician. Students who need a physical to obtain working papers are also welcome to have physicals at this time.

In addition, the district provides annual vision screening for students in grades K-3, 5, 7, 10 and new entrants; hearing screening for students in grades K, 1, 3, 5, 7, 10 and new entrants and scoliosis screening for pupils in grades 5 through 9.
