Nurses' Office information

 

School and Sports Physicals/10th Grade Physicals

If your child has a physical exam with their physician we would appreciate a copy from your physician on our GHS School/Sport Physical Examination Form. In addition, State Education Law requires that all students entering 10th grade be given a physical exam by a physician sometime within that year. Again, please have your physician complete the School/Sport Physical Examination Form and either fax the completed form directly to the Nurses’ Office at 861-5151 or mail to GHS, attention Nurses’ Office. Extra physical forms can be obtained either from the GHS Nurses’ Office or downloaded from this website. For those who prefer to have their child have a physical at the High School Nurses’ Office, physicals are offered four times a year, and the dates are posted on the website. There is no charge, but an appointment is needed unless specified.

Sports Process

When participating in sports at the High School, each student must pick up a sports’ packet (consisting of three forms) each season from the Nurse’s Office. The packet needs to be completed and turned in to the Nurse’s Office as soon as possible. Students must also have a current physical in order to participate in sports. School physicals are given at the High School every sport season, and are good for 12 months.

Injuries

Physical Education at the High School level is an accredited course. If a student is injured in school or out of school and has been seen by an M.D., a note is required to inform the Nurse’s Office of the student’s diagnosis and limitations. The High School has an adaptive PE teacher who adapts the student’s personal abilities to their injuries. A PE Modification Form is available in the Nurse’s Office for the M.D. to complete.

Leaving School Due to Illness

When a student needs to leave school due to illness, they must come to the Nurse’s Office in order to be dismissed. The School Nurse will contact the student’s parent for permission for the student to drive home or to be picked up at the Nurse’s Office. To get to the Nurse’s Office: turn down the driveway closest to Depot Road (or the third driveway from Route 146); go over the first speed bump then take the first left into the parking; the Nurse’s Office is in the corner by the exit doors. For your convenience, there is a reserved parking space for the health office so you won’t even have to get out of your car. As soon as the student identifies you, he/she will be sent out.

Medication Policy

Please call the High School Nurse’s Office for information on medication administration during the school day. Students are not allowed to carry medications in the High School. The only exceptions are inhalers and epipens with MD and parent permission. For those students who use inhalers, please make sure their prescriptions are filled and the student has their inhaler with them at all times, especially for PE and sports.
