Spring sports sign-ups begin Jan. 17
Jan. 10, 2016—Are you interested in playing a sport this spring? If so, sign-ups begin on Jan. 17!
Spring sports start March 6, and include boys/girls track and field, boys/girls lacrosse, softball, baseball, and boys tennis and unified basketball.
Students interested in playing a spring sport should pick up a sport sign-up packet from the high school nurses' office and return the completed packet to the nurses' office as soon as possible. The sport packet includes three forms: the School and Sport Physical Form, the Athletic Form and the Health History Form—all of which are also available online under "Athletics" on the "District Information" quick link. Students will not be able to try out or participate in practices without a sports packet cleared by the Nurse’s Office.
Students needing sports physicals should sign-up in advance with the high school nurses’ office for either:
• Tuesday, February 14 from 3:30 – 5:30 p.m.
• Tuesday, February 28 from 3:30 – 5:30 p.m.
Please note: Appointments may take up to one hour, so please plan accordingly. Physicals are also open to all students for working papers, tenth grade physicals, and new student physicals. If applicable, please bring glasses or wear contacts for the eye exam. Students may have physicals done by their own doctor, but the physical must be completed on the GHS physical form and reviewed by the school nurse before a student can sign-up for a sport or attend practice. Students will not be able to try out or participate in practices without a sports packet cleared by the Nurse’s Office. The Department of Athletics form and the Health History form should be handed in ASAP; the Physical Examination form must be handed in before try-outs/practice starts.
Please note: If a student is seen by a doctor for any injury, accident or serious illness, the parent/guardian must notify the school nurse. This is required whether or not the injury/illness occurred on school grounds. Upon his/her return from an injury, the student must have either a written release or restriction from the attending physician, which should be given to the school nurse for review; a copy will be sent to the Coach, and the original kept in the student’s health records.
