Dear Parents and Guardians,
As we prepare for another school year, the Guilderland Central School District Health Offices would like to thank you for your partnership in helping keep our students healthy, safe and ready to learn.
We hope you are enjoying your summer, but ahead of the 2026-27 school year please take a moment to review these important health reminders:
New York State Physical Examination Requirements
New York State requires a current physical examination for students entering or attending:
- Pre-K or Kindergarten
- Grades 1, 3, 5, 7, 9 and 11
- All new students entering the district
- Annually for interscholastic sports, working papers or when required by the Special Education Department
Physicals must be completed using the required New York State Health Examination Form and submitted to your child’s school health office as soon as possible. Physicals completed within the last 12 months are acceptable for the upcoming school year.
Required Immunizations
Please ensure your child’s immunizations are up to date before the start of school in September. Important middle and high school requirements include:
- Tdap Vaccine is required for all students entering Grade 6 (aged 11). Students must be in compliance within 14 days of turning 11.
- Meningococcal (MenACWY/Menactra or MenQuadfi) Vaccine is required for students entering Grades 7 and 12.
Immunization records may be submitted:
- By fax to your child’s school health office
- By email as a scanned document/PDF
- In person to your child’s school

We suggest you submit updated records as soon as vaccinations are completed to avoid delays at the start of school.
Medication Orders for the 2026-27 School Year
If your child will need medication during the school day next fall, new medication orders and parent permission forms must be provided each school year.
This includes:
- Prescription medications
- Emergency medications (EpiPens, inhalers, seizure medications, etc.)
- Over-the-counter medications
Medication orders must be signed by your child’s healthcare provider and parent/guardian before medications can be administered at school. Please also remember:
- Medications must be brought to school by an adult
- Medication must be in the original labeled container/pharmacy box
We encourage families to schedule summer pediatrician appointments early, as healthcare offices often become very busy in August.
