As we prepare for the 2025–26 school year, all families are reminded of the New York State health requirements for students in grades K–12. Compliance with these regulations ensures a safe and healthy school environment for all students
Immunization Requirements
New York State law mandates that all students must be up-to-date on required vaccinations to attend school. Only medical exemptions with documentation are allowed — nonmedical exemptions (including religious) are not accepted.
Required vaccinations include:
- Diphtheria, Tetanus, and Pertussis (DTaP/Tdap)
- Polio (IPV)
- Measles, Mumps, and Rubella (MMR)
- Hepatitis B
- Varicella (Chickenpox)
- Meningococcal Conjugate (MenACWY)
Important notes:
- Grade 6 students (Age 11 and Older): must receive the Tdap booster.
- Grade 7 students: 1 dose of meningococcal vaccine is required.
- Grade 12 students: 2 doses of meningococcal vaccine are required (or 1 dose if given on or after age 16).
Immunization records are due within 14 days of the first day of school. Students who are not in compliance with vaccine requirements within this time frame are subject to exclusion from school until records are received.

Physical Examination Requirements
Physicals are required by NYS law at the following times:
- When entering the school district for the first time
- Grades K, 1, 3, 5, 7, 9, and 11
- Prior to participation in interscholastic sports (annually)
- When applying for working papers (employment certificates)
- When requested as part of a special education evaluation
Physicals are due within 30 days of the start of school. Forms must be completed by a licensed New York State healthcare provider and submitted using the NYS School Health Examination Form.
Submitting Health Documentation
All immunization records and physical forms should be submitted to the school nurse by the applicable deadlines. You may:
- Drop them off in person
- Fax them to the school
- Mail them to your child’s school health office
